We are seeking a Community Navigator to provide practical, front-line assistance to residents accessing social and community services. This role helps individuals complete applications, understand options, and connect with needed supports.
What are we looking for?
- Post-secondary education in Social Services, Public Health, Human Services, Community Development, or similar
- Experience working in social services, case management, or a related role
- Strong understanding of community resources, social assistance programs, housing supports, and health services
- Ability to work respectfully and effectively with individuals experiencing complex or sensitive life circumstances
- Strong communication, interpersonal, and relationship-building skills
- Excellent organizational skills with attention to detail in documentation and follow-up
- Ability to work independently while collaborating with community partners and service providers
- Valid Class G driver’s license required
- Vulnerable Sector Check required prior to commencement of employment
What will you be doing?
- Providing direct support to individuals seeking access to social services, housing, and physical or mental health supports
- Assessing needs through intake conversations and identifying appropriate community resources and referral pathways
- Connecting individuals with service providers and following up to support successful navigation
- Assisting clients with completing applications, forms, and documentation
- Maintaining accurate, confidential case notes and records
- Supporting individuals facing complex personal, social, or economic challenges
- Acting as a liaison between residents and service providers
- Staying current on available community programs, eligibility criteria, and system changes
- Supporting outreach and awareness of available services and resources within the community
